New Patients

General Policies

Please check-in at the receptionist desk when you arrive for your appointment. For your first visit to our office, please come 30 minutes before your appointment to fill out patient information forms. We will also need information about your insurance coverage and medications. Please bring your insurance card and driver’s license with you to every appointment. Should you have any information change please let us know so we can update your records.

We sincerely try to stay on schedule. Unfortunately unforeseen emergencies arise. We ask for your patience when these delays occur.

As a courtesy to us and our other patients, we ask that you please give 24-hour notice should you need to cancel or reschedule your appointment.

No-Show Policy

Please note, patients who fail to show up for a scheduled appointment without at least 24-hours notice more than three times may be asked to transfer their care to another provider.

Insurance Acceptance

We accept most major insurance, including Medicare and Medicaid. You may call our office to verify that we accept your insurance plan.

We also offer a wide range of assistance programs.
